Privacy Policy

We value your privacy and all information collected is treated as confidential information. We handle your information in compliance with our obligations under the Privacy Act 1988 and Health Records and Information Privacy Act 2002 (to the extent applicable). We use your information only for the purpose of providing you with quality care in connection with the dietetic services we offer. In some circumstances, we will disclose selected information to other health providers (i.e. medical practitioners, specialists or other allied health professionals) if it directly relates to your care or where required by law. Your information is securely stored and only accessible by us for the abovementioned purpose. You can request access to your personal information or request we correct the information at any time. 

 

Rescheduling / Cancellation Policy

All prepaid appointments are non-refundable. If you are unable to keep your appointment, please provide us with at least 24 hours’ notice so your appointment can be rescheduled or cancelled. If you miss an appointment without notifying your practitioner or cancel your appointment with less than 24 hours notice, the prepaid session will be forfeited and no refunds will be made available. If cancellation / rescheduling with less than 24 hrs notice is due to an emergency, please contact your practitioner as soon as able to reschedule your appointment. Application of our rescheduling / cancellation policy remains subject to the discretion of your practitioner at all times.

 

Please note that if you are more than 15 minutes late to your scheduled Telehealth appointment, your appointment is deemed cancelled and no refunds/rescheduling will be made unless under the discretion of your practitioner. 

 

Payment Policy

Our fees are non-negotiable and non-refundable. You are responsible for making full payment at the time of service and if you have prepaid for packages, all unused sessions are non-refundable and forfeited if they are not used within 8 weeks of purchasing the program or unless otherwise notified by your practitioner. 

 

CLIENT CONSENT

By ticking and signing on the checkout page:

  •  I declare that the information that I have provided on this form is true and accurate at the time of signing. 
  •  I acknowledge and agree to the Terms & Conditions outlined above.
  •  I consent to my dietitian (Danica Vaz or other relevant practitioner) collecting and storing my personal and health information for the purpose of providing dietetic care.
  •  I consent to my dietitian (Danica Vaz or other relevant practitioner) to share my personal, health or confidential information (to the extent relevant)  with my GP or other relevant health professionals involved in my care.
  •  I understand that my credit card details will be kept securely on file for future transactions on my account. I authorise my card to be charged for dietetic services agreed with my dietitian (Danica Vaz or other relevant practitioner) including for purchases, payment plan subscriptions and late fees. 
  •  I consent to receiving newsletters and marketing emails from Danica Vaz and Dietitian Danica and I am aware that I can unsubscribe at any time.
